The incidence of small bowel perforation in Crohn's disease (CD) is rare but difficult to diagnose pre-operatively in patients with undiagnosed CD.Although the incidence in Japan of perforation in CD has been rare, it has been the presenting sign in about 70% of cases. In this case CD was diagnosed after free perforation. We suspected IBD from CT and confirmed it by surgery. Since the incidence of CD is increasing, CD should be included in the differential diagnosis of perforated peritonitis.
