Abstract

Aim: To identify the reflection support in clinical judgments offered by certified nurse specialists in critical care nursing (CCNS) to personnel that are newly transferred to emergency intensive care units (EICU) from other departments.

Method: An invitation to participate was sent by mail to a list of certified nurse specialists registered with the Japanese Nursing Association. Data were then collected from semi-structured interviews with 10 respondents who agreed to participate. Subsequently, a qualitative data analysis of findings was conducted leveraging the steps as described by Berelson (1952).

Results: The reflection support for clinical judgment provided by CCNSs was classified into three categories for reflection-in-practice: “explaining perspectives to detect abnormalities in the life support system function and changes in patient condition,” “conveying how to evaluate patient condition based on multiple data of patients,” and “explaining intervention strategies to predict patient conditions and prevent critical situations. ”. For reflection-on-practice, three categories were extracted: “conveying the way of thinking and the care methods that were not conveyed during practice, ” “deriving meanings from experienced cases that can be applied to future nursing care,” and “accepting the transferee’s thoughts and practices to foster professional growth.”

Conclusion: The CCNSs provided reflection support to nurses who were transferred to the EICU by sequentially verbalizing their thoughts and practices.
