Clinical study of failed implant cases in our department

Abstract
Dental implant treatment cases have been steadily increasing in recent years. Reported here arefindings of a clinical investigation of patients referred for complications following such treatment.This study examined details of 198 patients with implant complications referred to the Departmentof Oral and Maxillofacial Reconstructive Surgery at Hiroshima University Hospital over the 17-year period from January 2006 and December 2022. Each had undergone implant treatment atanother institution. Eight parameters were analyzed; (1) medical history, (2) sex and age, (3) annualnumber of new patients at prior institution, (4) referral source, (5) itemization of complications, (6)prosthetic form in cases requiring implant removal, (7) treatments performed at our department,and (8) sex and age distribution of patients with peri-implantitis requiring removal. Hypertensionwas the most common comorbidity and observed in 28 cases. Patient age ranged from 26 to 91 years(mean 65.9 years), and there were 73 males and 125 females. The greatest number of new referralswas noted in 2018 and comprised 27 cases. Regarding referral source, 115 patients were referred bya dental clinic and 65 from another department within our hospital, with peri-implantitis the mostfrequent condition leading to referral (n=149), followed by implant body fracture (n=21). Among casesrequiring implant removal, the most common prosthetic design was splinted crowns, with that notedin the left maxillary molar region in 21 and the left mandibular molar region in 21. The most commonprocedure performed by our department for all cases was implant body removal (n=110). Analysisof patients who underwent removal due to peri-implantitis revealed a large number women aged 60years and older.
