Japanese Model of Disease Management Matching the Era of Particular Health Check-up/Healthcare Instruction

Abstract
 The Amended Medical Care Law has been determined and included “Tokutei Kenshin” (particular health check-up) for lifestyle disease on every 40-74 year-old citizens followed by particular healthcare education system from April 2008. Although insurers will have to make it on maximum 57 millions of covered people, they will be able to accumulate and utilize standardized physical and lifestytle data.
 We have been conducting a disease management ‘Carna project’ for diabetes mellitus secondary/tertiary prevention since 2002. We newly developed a disease management program for primary prevention of life style disease to accept outsourcing by insurers, and now we can support seamless service from primary to tertiary prevention for diabetes mellitus. We emphasize (1) quality management by development of an outcome-oriented critical pathway and IT system which supports operator’s workflow, (2) correspondence with customer’s variation by the critical pathway which supports optimal personalized care, and (3) adherence to medical related laws and law of privacy protection. We are conducting a verification project on the medical outcomes and the business model with about 300 insured people from several health insurance societies until the end of 2007. Primary health instruction had a better effect on eating behavior compared to other lifestyle behavior.
