Abstract
 ICD–10 is used widely for the world. Because of the necessity of revision in accordance with recent medical progress and changes of social system, the World Health Organization (WHO) started the revision process for ICD–11 in 2007. We have reported the current status of the revision and focused on the domestic committee in the present report. WHO has established the Topical Advisory Group (TAG) in twelve medical domains involving internal medicine, mental health, injury and external causes, rare disease, neoplasms, maternal and prenatal, ophthalmology, health information modeling, dermatology and musculoskeletal, dentistry, neurology. Japan took a responsibility on the internal medicine domain, declaring active participation in the revision process. Therefore, Japan has established a domestic committee to support the Japanese chairman and TAG for internal medicine. We have a lot of problems, such as the language translation, improvement of information model, intake of ontology etc.
