Background: Urinary sediment analysis may reveal adenocarcinomas including primary tumor and metastatic tumor, as well as contaminations. Herein, we report a case in which a rare adenocarcinoma was detected in a patient who underwent bladder augmentation using the ileum 53 years ago. Case: The patient was a man in his 70s. Fifty-three years ago, he underwent left nephrectomy and bladder augmentation using the ileum for renal tuberculosis and bladder tuberculosis. Urinalysis revealed atypical cells, and histopathological analysis of a cystoscopic biopsy specimen performed afterwards confirmed adenocarcinoma derived from the ileum. Conclusion: We encountered a rare case of adenocarcinoma detected by urinary sediment examination. Currently, bladder augmentation using the ileum is rarely performed. However, as in this case, many patients develop adenocarcinoma after a long period of time, so we believe that we may still encounter such cases in today examinations. Therefore, after it is important to understand its clinical background and morphological features, performing microscopic examination.
