Objectives: This study aimed to clarify the angle on prevention of long-term care management studied by a public health nurse, a social worker and a chief care manager (three types of professionals) at a community general support center and to show the effects and challenges of the support conducted based on the angle above.
Methods: A study of 17 cases for elderly persons whom the long-term care insurance system in Japan categorized as persons requiring support was carried out by the three types of professionals, and the angle confirmed its importance for prevention of long-term care management was extracted. After the support, the three types of professionals evaluated the performance based on the angle which they made sure of its importance, and analyzed the effects and challenges.
Results: The angle confirmed its importance on prevention of long-term care management was divided into 26 subcategories and 6 categories as “supporting cooperatively with supporters,” ”adjusting support with prospects for making support system,” “making comprehensive assessment,” “supporting to maintain the current endeavor,” “advising from prevention of long-term care’s point of view,” “supporting to live up to elderly persons and their family’s desire.”
Conclusions: Results clarified that it is effective for prevention of long-term care to support independent activities of elderly persons who require support and to coordinate with specialists and communal supporters to assist elderly persons properly. Meanwhile, the results also revealed that there is the need for a consultation system and for suggestions about specific attempt with technical knowledge to allow elderly persons with chronic illness to live with ease, and the need to establish and then, grow up the system to care about elderly persons as a base to build a communal system of apprehension for elderly persons.
