Objective: To clarify current practices in checking methods used by nurses during injection drug preparation in acute care hospitals and to examine the perceptions of ward nurses and medical safety managers regarding characteristics that hinder effective checking behavior.
Methods: A nationwide questionnaire survey was conducted among ward nurses and medical safety managers at 126 hospitals.
Results: Responses were received from 1,190 ward nurses and 100 medical safety managers. Over 14 different checking methods for drug preparation were reported. Two-person checking was used in 64.4% of verification procedures (33.5% independent and 30.9% dependent) and in 95.2% of calculation procedures (30.1% independent and 65.1% dependent). Perceptions of inhibiting characteristics including lack of accountability, increased time requirements, and workflow interruptions—varied by checking method and differed between ward nurses and medical safety managers.
Conclusion: Robust error prevention requires adopting checking methods that minimize characteristics inhibiting effective checking behavior.
