We encountered an end-knot where the contralateral leg wire became entangled with a stiff wire during surgery using AFX2. The patient was an 89-year-old man who underwent a 2-stage surgery for bilateral common iliac artery aneurysms and a left internal iliac artery aneurysm. First, coil embolization of the left internal iliac artery was performed, followed by stent graft insertion (endovascular aneurysm repair [EVAR]) using AFX2. An end-knot phenomenon occurred when the AFX2 was inserted, and the snare caught the contralateral leg wire. However, eventually end-knot phenomenon was released, and the surgery was completed.
