Abstract : Microbial invasion is conceivably the major and the most common cause of persistent periapical lesions. It is thought that periradicular damage caused by mechanical irritation such as over-instrumentation, chemical irritation such as irrigants and intracanal medications is coupled with microbial infection to develop and perpetuate severe inflammatory reactions. So, it seems desirable to avoid unnecessary irritation in root canal treatment, especially in the therapy of persistent periapical lesions. Following completion of chemo-mechanical preparation and drainage, in our clinics, calcium-hydroxide containing agents are placed into the root canal and maintained for more than 4 months without the change of agents in the case of persistent periapical lesions. After assessing the stage of periapical healing clinically and radiographically, the root canals are obturated using a vertical condensation method. Even in the case of persistent periapical lesions, the application of the methods mentioned above result in favorable healing as well as in fewer appointments to complete the root canal therapy.
