A 75-year-old man was presented to the internal medicine department with swelling of his left lower abdomen dur- ing urination. On arrival, the patient’s vital signs were normal. A bulge was observed when pressure was applied to the abdomen; therefore, an inguinal hernia was suspected. However, the bulge was found to be slightly more cepha- lad than inguinal; therefore, it could not be explained by an inguinal hernia. Computed tomography of the abdomen revealed an enlarged prostate and a diverticulum in the upper left of the urinary bladder. The patient was diagnosed with a bladder diverticulum induced by increased bladder pressure due to prostate enlargement. When swelling in the lower abdomen is observed on compression of the abdomen, bladder diverticulum should be suspected.
