Abstract
These days, medical science has remarkably progressed, and social surroundings are changing. Since Long-term Care Insurance System was enforced in 2000, people with disease or disability who live in the community are increasing.
Patients Support System is essential to improve patients' QOL and support patients, giving respect to their own principle. In order to strengthen the cooperation among health, medical and welfare organizations and play a responsible role by building a support system, we have promoted the liaison between Tokushima University Hospital and Tokushima Health Center.
In 1996, we started the liaison system. At the beginning, our activity was mainly directed to providing child health care and supporting mothers. We held case conferences about sick or low-weight-birth children once a month. At that time, participants were pediatric doctors in the University Hospital and public health nurses of health center.
Later, in 1998, “Consultation Room For Continuous Medical and Nursing Services” was established in Tokushima University Hospital, and the room has been identified as the section of information and arrangement about health/medical/welfare cooperation. Since then, the condition has changed to be better for gathering workers, and nurses of University Hospital began to join the conference. In addition, weinstituted “Arrangement For The Liaison Meeting between Tokushima University Hospital and Tokushima Health Center”. Now, in the liaison meeting, a variety of cases are discussed in order to support patients and their families who live in their community. We exchange the information about cases and health/medical/welfare resources that is useful. This time, we report the progress, the method, and the effectiveness of 6 years' trial.
