A Patients with Type 2 Amiodarone-Induced Thyrotoxicosis (AIT2) and VT Storm

Thirty six years old man with dilated cardiomyopathy was already received anti-arrhythmic therapy (amiodarone 200 mg/day, sotacol 160 mg/day) and bi-ventricular pacing (CRT-D). His ventricular tachycardia (VT) was hard to control, and he needed repeated hospitalization. When cholecystitis triggered VT storm, long term management in CCU with tranquillizer was needed. Radiofrequency ablation for VT was failed. Finally, increasing amiodarone dosage from 200 mg/day to 400 mg/day could control VT, and he could get out of hospital. However, two months later, he developed type 2 amiodarone-induced thyrotoxicosis (AIT2; fT4>6 ng/ml, TSH<0.01 µU/ml), he worried about VT again. Corticosteroid therapy (predonisolone 30 mg/day) for AIT2 and 2nd radiofrequency ablation successfully control VT.
