We aimed to compare the clinical efficacy of I-Root BP Putty and calcium hydroxide for indirect pulp capping of deeply carious permanent teeth. In this prospective study, 180 patients were assigned to an experimental group (I-Root BP Putty, n=90) or control group (calcium hydroxide, n=90. Pain was assessed using Visual Analogue Scale (VAS) scores before treatment and at 1 week, 1 month, and 3 months. Clinical success rates were evaluated, and multivariate logistic regression was used to identify associated factors. Subgroup analysis by tooth position was also performed. Baseline characteristics were comparable between groups. VAS scores decreased significantly after treatment in both groups (P<0.001). The experimental group had significantly lower VAS scores than those of the control group at 1 week and 1 month after pulp capping (P<0.05), but a comparable VAS score to the control group at 3 months after pulp capping (P>0.05). According to multivariate Logistic regression analysis, in contrast with calcium hydroxide, I-Root BP Putty for indirect pulp capping significantly enhanced the treatment success rate (OR=2.73, P=0.047). The clinical success rate was higher in the experimental group than in the control group in the anterior, premolar and molar teeth, but the difference was of no statistical significance (P>0.05). I-Root BP Putty yields favorable clinical outcomes in indirect pulp capping of deeply carious permanent teeth, together with advantages in reducing early postoperative pain and enhancing the treatment success rate.
