Abstract

We herein report a case of isolated oculomotor nerve paralysis due to sphenoid sinus mucocele. A 65-year-old man developed right-side ptosis and deplopia. Despite his right-side ptosis and disturbance of the upward, inward and downward ocular movement an ophthalmological examination showed neither visual impairment nor pupil dilation. Computed tomography (CT) and magnetic resonance imaging (MRI) showed lateral smooth-margined sphenoid sinus lesions compressing the right cavernous sinus. The patient was diagnosed with sphenoid sinus mucocele and underwent endonasal endoscopic surgery under general anesthesia. All symptoms disappeared within two months postoperatively.
