We herein report three cases of sinusitis with intracranial complications. All cases underwent endoscopic sinus surgery with a percutaneous approach for drainage. Two cases recovered without neurological sequela, but one case died because it took a time before initial treatment was performed. An early diagnosis and treatments including empiric antibiotic therapy and surgical treatment should be recommended. In addition, surgical treatment combined with endoscopic sinus surgery and neurosurgical procedure for drainage should be considered for sinusitis with intracranial complications.
