Twenty-six cases with malignant submandibular gland tumors were retrospectively investigated regarding the treatment modalities and treatment outcome. As a result, the overall 5-year survival rate as determined by the Kaplan-Meier method was 47%. With respect to the major histopathologic types, the 5-year survival rate of adenoid cystic carcinoma was 60%, while the same rate for adenocarcinoma and mucoepidermoid carcinoma was 50% and 40%, respectively. The frequency of local recurrence was 50%, and that for T3T4 cases were especially high at a rate of 67%. The frequency of neck recurrence in N0 cases who had not undergone a neck dissection was 29%. These results suggested that a radical resection and postoperative radiotherapy might be necessary for primary lesions while, in addition, a neck dissection, including at least a SOND (supraomohyoid neck dissection) may be preferable for such N0 cases.
