    We encountered two cases of Wallenberg's syndrome (lateral medullary infarction) and were able to cure both cases. The first case showed a typical crossed dissociated sensitivity disturbance, but the second case as ipsilateral. In these cases the MRI images generally showed a low density and a T2 image showed a high density on the postlateral medullary obolongata. However, in the second case, the lesion occurred only on the right side of the posteroinferior cerebellar hemisphere but not on the medulla oblongata. We speculate that this is because the medullary infarction was incomplete. As a result, the patient made a relatively rapid recovery. In the reported cases of Wallenberg's syndrome, the patients often complained of vertigo as well as difficulty in swallowing. In addition, many of them tend to consult not only an internist but also an otolaryngologist. Hence, we believe that in the future otolaryngologists should increase their knowledge regarding this syndrome in order to better treat such patients.

    An abnormal increment of the amplitude in electroneuronography (ENoG) could be observed on the affected side of intratemporal nerve palsy. In order to study both the significance and mechanism of this phenomenon, the ENoG findings of facial nerve palsy cases were recorded. As a result, 10% of the findings showed a great amplitude potential. In animal experiments, 35% of the guinea pigs also showed a great amplitude potential in the facial muscles on the affected side. An electromicroscopic examination of the facial nerve at the compressed site revealed a separation of the myelin sheath laminae. Due to changes in the resistance of the myelin sheath, the iron dynamic state, which is related to the excitability of the nerve also changed and showed hyperexcitability.

    We devised a new over-lay method for simple in-office myringoplasty using a skin graft from the auricle for perforations following tube insertion for secretory otitis media. After removing the epithelia around the perforation following surface anesthesia of the eardrum, a skin graft from the auricle posterior from the externel ear canal is placed on the eardrum. This technique was used on five patients with perforations following tube insertion for secretory otitis media. A successful closure of the perforations was obtained in 4 out of 5 patients (a success rate of 80,0%) by the intial surgery and the remaining 1 reperforation case could be successfully closed with a second operation. As a result all perforations could be closed using our method by either the first or second operation. To prevent a transmeatal infection and to resolve any handicap due to a perforation of the eardrum, it is important to close such perforations at the same time that the tubal function is relieved. Since the size of the perforations following tube insertion for secretory otitis media is generally small, such small perforations are a good indication for in-office myringoplasty. In the future, we plan to study the usefulness of our method by further increasing the number of cases.

    Basaloid squamous cell carcinoma (BSCC), which was first described by Wain et al. in 1986, is a histologically distinctive variant of squamous cell carcinoma. It is a biologically virulent neoplasm with a propensity for nodal as well as systemic metastasis. While it typically occurs in the upper aerodigestive tract, BSCC in the epipharynx is quite rare with, to the best of our knowledge, only five previously reported cases. Our case was a 75-year-old man who presented with a left neck mass, left hearing impairment, and a sensation of fullness in the left ear. The treatment approach we chose was surgery, radiotherapy and adjunct chemotherapy. Although we could control both local and lymph node metastasis, the patient died of multiple lung matastases 25 months after the patient first visited our hospital.

    A very rare case of lipoma of the parotid gland is reported. The patient was a 71-year-old male who complained of a painless swelling in his right intra-auricular region. Radiological imaging by CT scanning and MRI revealed a tumor in the deep lobe of the right parotid gland, extending to the garapharyngeal space. The pathological nature of the tumor was very difficult to diagnose based on the findings of a preoperative fine needle biopsy, due to uncertainty regarding the location of the needle point at biopsy in relation to the tumor. The interoperative pathological diagnosis was useful in determining the appropriate operative course. The tumor was totally removed, together with the deep lobe of the parotid gland. Temporary facial paralysis was encountered postoperatively, but later resolved satisfactorily. This case is only the fifth in the world to be reported in the literature.

    It is generally accepted that sialolithiasis of the parotid gland is relativery rare. We herein report a case with sialolithiasis in the left parotid duct. A 63-year-old female presented with left cheek swelling of 2 weeks duration. Examinations shouwed swelling of the left parotid gland and a palpable small hard mass in the parotid duct. Ultrasonography using a transcutaneous probe and peroral probe revealed 2 high echoic masses with an acoustic shadow. To extract the stone, the oral approach was employed. The postoperative course was uneventful. Ultrasonography can search for stones from every portion of the parotid gland and duct without interfering with an artificial tooth. Based on the above findings, this method is considered to be safe and effective.

    Twenty-six cases with malignant submandibular gland tumors were retrospectively investigated regarding the treatment modalities and treatment outcome. As a result, the overall 5-year survival rate as determined by the Kaplan-Meier method was 47%. With respect to the major histopathologic types, the 5-year survival rate of adenoid cystic carcinoma was 60%, while the same rate for adenocarcinoma and mucoepidermoid carcinoma was 50% and 40%, respectively. The frequency of local recurrence was 50%, and that for T3T4 cases were especially high at a rate of 67%. The frequency of neck recurrence in N0 cases who had not undergone a neck dissection was 29%. These results suggested that a radical resection and postoperative radiotherapy might be necessary for primary lesions while, in addition, a neck dissection, including at least a SOND (supraomohyoid neck dissection) may be preferable for such N0 cases.

    To determine the utility of dynamic magnetic resonance (MR) imaging in evaluating parotid tumors, dyanamic MR imaging was perfomed in 10 parotid tumors. These included 8 benign and 2 malignant tumors, in which the histopathological diagnosis was established based on examinations of surgical specimens. Five types of time versus signal intensity curves were obtained: type A, curve peaks 0-30 seconds after the administration of contrast material; type B, curve peaks at 30-60 seconds; type C, curve peaks at 60-210 seconds; type D, a gradual upward slope; and type E, flat. A type A curve was specific to Warthin tumors, and a type D curve was specific to pleomorphic adenoma. Malignant tumors showed a type B curve. Dyanamic MRI was thus thought to be useful in predicting the components of parotid tumors.

    We researched the QOL of laryngectomized patients (26) who belonged to the Kinseikai society by questionnaire. Almost all patients (18 of 25 answers) used esophageal speech. All 6 patents who were 70 years of age or older at the time of laryngectomy were not able to achieve esophageal speech. All good speakers with esophageal speech could speak between 1 and 6 months after speech lessons. Eight of 11 answers on the advantages of esophageal speech showed self-confidence while their characters were diplomatic and aggressive. Moreover, esophageal speech was also considered to improve the olfactory function.

    A case of neurinoma in the nasal cavity is herein reported. The patient was a 57-year-old man who presented with a nasal obstruction of 2 years duration. An excision biopsy of the mass indicated a neurinoma based on the histological findings. The tumor was completely removed using a lateral rhinotomy under general anesthesia. The main mass was located in the right nasal cavity and extended to the epipharynx. The post-operative course was uneventful and the patient was discharged 9 days after surgery. Sixty-four cases of neurinoma in the nasal cavity and the paranasal sinus have been reported in the literature. The origin of the nerve and/or nerve fibers is not usually determined in such cases. We too were unable to identify the tumor origin in our case. Both recurrence and malignant changes in these tumors have occasionally been reported. Therefore, a complete surgical extirpation is considered to be necessary for the successfull treatment of such cases.

    A new allergen extract from Japanese cedar pollen (JCP) standardized by the content of Cry j 1, also called TO-194, has recently been produced. In this study we examined its allergenic potency in comparison to that of the standard cedar pollen extract established by the Allergen Standardization Committee, the Japanese Society of Allergology using the intradermal titration technique. Each extract in 3-fold serial dilutions (TO-194 and the standard cedar pollen extract) was simultaneously injected in each of 71 patients allergic to JCP and then the threshold values for positive reactions were obtained regarding their potency. Sixty of 71 patients could be completely followed and analyzed. As a result, no difference was found between the two extracts. No adverse side effects were seen in 70 patients analyzed on the first day and the next three days. Based on these findings, TO -194 is thus considered to be a standardized and useful extract for the treatment of patients with Japanese cedar pollinosis and for the comparison studies of treatment results at different clinics.

    Currently the retained extract from Japanese cedar pollen (JCP), a product manufactured by Torii Pharmaceutical Co., Ltd., is widely used in Japan for the treatment of patients allergic to JCP. However, it is not standardized, yet. Recently a new standardized allergen extract from JCP (TO-194) has been produced. The purpose of this study is to compare the allergenic potency of each extract. The potency was assayed by the intradermal titration technique; each extract in 3-fold serial dilutions was injected simultaneously in 50 patients with Japanese cedar pollinosis, and the threshold values for positive reactions were employed for the analysis in 44 patients. As a result, TO -194 was found to be more potent than the currently retained extract. In addition, no adverse side effects were seen in either group (48 patients in all were analized) on the first day and the next three days. Based on the above findings, TO -194 is thus considered to be preferable to the currently retained extract for immunotherapy in patients allergic to JCP.

    A study was conducted on 20 subjects with Japanese cedar pollinosis to confirm the safety of standardized Japanese cedar pollen extract (TO-194: 2,000 JAU/ml) in specific immunotherapy. None of the subjects exhibited any serious adverse side effects, however, mild adverse side effects, for which a causative relationship with TO-194 could not be ruled out, were observed in 2 of 20 subjects. The safety rete of TO-194 was therefore 90% (95% confidence limits: 68.3-98.8%). The 2 cases exhibiting adverse side effects comprised of 1 case with nasal secretion and 1 case with eye itching. Both symptoms disappeared before the treatment with maintenance dose, and the injectional dose could be increased without incident in both cases. As a result, specific immunotherapy using standardized Japanese cedar pollen extract (TO-194) is considered to be safe since it caused no signifficant problems in this study.
