A 55-year-old female was referred to our hospital because of an abnomal shadow on a chest X-ray film which had been found in a medical examination. CT scan revealed a tumor in the middle mediastinum which seemed to be connected to the right lobe of the thyroid. Thyroidal scintigraphy showed a slight accumulation of radioisotope which led to a diagnosis of mediastinal goiter.
During surgery the tumor was found to be attached to the cervical thyroid only with fibrous tissues but not connected. The tumor adhering to the superior vena cava was safely resected under a collar incision of the neck and median sternotomy. The pathological diagnosis was adenoma tous goiter. Following Rives' classification, this tumor was thought to be an aberrant mediastinal gaiter.
