We report the onset of central retinal artery occlusion during endoscopic sinus surgery performed under general anesthesia. A 70-year old male underwent bilateral endoscopic sinus surgery for recurrent chronic sinusitis with nasal polyps after a 50 year history of nasal obstruction, nasal discharge and disturbance of smear. Patient noticed sudden onset of unilateral blindness in left eye after the surgery. We initially considered and treated it as intraorbital side injury in the sinus surgery until ophthalmological examination on the next day. The critical golden period for treatment of central retinal artery occlusion is within 2 hours. We failed to effectively treat the blindness in this case. The results might have been better if we had made the correct diagnosis within the critical golden period of this disease.
