We described our technique of rhinoplasty via an external approach through transcolumellar and marginal incision, and which allowed cartilage, septum and nasal bone to be visualized for greater surgical accuracy. Three patients with shifted nose,10 patients with posttraumatic septal deviation,3 patients with posttraumatic saddle nose,1 patient with Binder' s syndrome, and 15 patients with cleft lip nasal deformity were operated with this approach and satisfactory results were obtained. Compared with the conventional intranasal approach, our approach has allows better direct vision and easier surgical maneuver. The disadvantages are unsightly columellar scar and more radical surgical maneuver. However, a careful surgical maneuver overcomes these disadvantages.
