    We devised a series of tasks as therapy for some types of vertigo. The task are as follows: 1) keeping the head in a position that elicits vertigo,2) head movements beginning in the sitting position,3) a short rest,4) body trunk movements,5) clockwise and counterclockwise body rotations while standing with eyes closed,7) walking forwards and backwards with eyes closed.
    We obtained favo r able results with this therapy in one of ten patients with Meniere's disease, all seven patients with benign paroxysmal positional vertigo and six of ten patients with vertebro-basilar artery insufficiency. Our home therapy is recommended for all patients with benign paroxysmal positional vertigo and selected patients with vertebrobasilar artery insufficiency.

    We investigated the effects of repeated stimulations for a few minutes on post-rotatory nystagmus. The subject sat on a rotatory chair that was suddenly decelerated following rotation at a constant angular velocity of 60 deg/s. The stimulation was given 3 times each in the leftward and rightward directions with a five minutes rest between trials. The subjects also recieved the stimulation following alcohol ingestion (0.5 ml/kg) and a 30-minutes rest. Post-rotatory nystagmus was recorded by electronystagmography and its duration and frequency were calculated.
    The duration of post-rotatory nystagmus and its frequency were decreased slightly with repetition of the rotatory stimulation. However, following alcohol ingestion, repeated stimulation had no effect on the duration and frequency of the elicited nystagmus.

    This study evaluated whether changes in ABR wave latencies can be predicted from the threshold audiogram in combination with age, sex and stimulation level by canonic correlation.
    The canonic correlation for 536 subjects was 0.66 for the first canonic pair. With all six canonic variables, only 32% of the raw ABR varience could be explained by the hearing loss. The second variable accounted for 22%, showing the greatest effect of hearing loss on the ABR. Analysis of component loading received that: 1) wave I latency correlated with the slope of the hearing loss between 2000 and 8000 Hz, with the severity of the hearing loss at 4000 and 8000 Hz, and with sex; 2) age, sex and low-frequency hearing loss correlated with the I-V and III-V interwave latencies.

    Body sway was analyzed in seven healthy subjects and three patients with central disorders. Power spectra were calculated by three different methods: 1) fast F ourier transform [FFT] with no smoothing procedures [degree of freedom 2],2) FFT with smoothing treatment [degree of freedom 26], and the maximum entropy method [MEM].
    Although the spectra obtained from FFT with no smoothing procedures varied widely, those calculated with the second two methods were very stable and allowed detection of abnormal patterns.

    We examined the influence of visual fields on upright standing by posturography in 19healthy adults. Each subject was placed on a stabilometer with the feet closed together under four conditions of visual fields: 1) eyes open,2) central area (30 degrees) of the visual fields open,3) central area (5 degrees) of the visual fields open,4) eyes closed.
    The central area of the visual field had a greater influence on postural control of vision than the peripheral retina when gazing at a target. The central retina of 5 degrees accounted for nearly 50% of all visual contributions to postural stabilization, and that of 30degrees about 70% of all visual contributions. This is more prominent in anterior-posterior component. Power spectra ranging from 0.2 to 0.8 Hz were closely related to vision, especially in latral sway component. A peak at 0.3 Hz was noted with eyes closed and central area of 5 degrees open.

    Sixty-two patients with parotid tumor were treated in our clinic during the past tm years. There were 35 males and 27 females. The peak incidence was observed in the sixth decade. Among them,24 were malignant. The most common tumor was pleomorphic adenoma (31 cases), whereas adenocarcinoma was the most common among the malignant tumor (7 cases). Pain, facial nerve paresis and adhesion to surrounding tissue were frequently observed in patients with malignant tumors.

    Two hundred and sixty-nine patients with laryngeal carcinoma were treated in our department between 1954 and 1985. The male to female ratio was about 20: 1. There were 166 glottic tumors (61.7%),86 supraglottic tumors (32.0%),11 subglottic tumors (4.1%). According to the TNM system of International Union Against Cancer (Union Internationale Contre Le Cancer),64 patients had stage 1 (23.8%),90 patients stage 2(33.5%),73 patients stage 3 (27.1%) and 42 patients stage 4 (15.6%).
    The 5-year survival rate in glottic tumor was 20.0 % (1955-1964),48.9%(1965-1974) and 67.1% (1975-1983). On the other hand, in supraglottic tumor the 5year survival rate was 14.3% (1955-1964),42.9% (1965-1974) and 57.7% (1975-1983).

    We analyzed the survival rates of 193 patients with maxillary cancers treated between 1955 and 1985. The majority of the cancer patients had stage III tumors, which accounted for about 66% of the total. The patients divided into three groups according to the period when treated: group A (1955 to 1961), group B (1962 to 1970) and group C (1971 to 1985). The survival rates were calculated according to therapeutic procedures.
    Crude 5-year survival rates for stage III were 0% in group A,48% in group B and 39%in group C. The most effective therapy was the combination of preoperative radiotherapy, operation and postoperative radiotherapy. Despite remarkable adva n ces in diagnostic techniques, little progress has been made in detecting advanced cancer.

    We investigated 44 children with adenoid vegetation who underwent adenotomy between 1985 and 1987.
    Their parents were surveyed by a quetionaire concerning their snoring, mouth breathing, hearing impairment and suspectibility to infection in 1988. Their mean age was 6 years. The mean hospitalization period was 8 days. The main symptoms of adenoid vegetation were snoring, difficulty in nasal breathing and nasal obstruction. The most serious complication was secretory otitis media.
    The improvement rates for snoring, mouth breathing, hearing impairment and suspectibility to infection after the adenotomy were 97.0%,74.3%,55.0% and 96.3%, respectively. The childlen's parents reported that the adenotomy was successful in 91.3%.

    A statistical study was made on foreign bodies in the esophagus in fifty-seven patients hospitalized in our clinic between 1979 and 1988. Twenty-four patients (42%) were under 5 years old, including under two years old. The objects were coins in 16 patients, and dental prostheses in 12. The obstruction was found at the first constriction of esophagus in 34patients. The object was removed by esophagoscopy in 46 patients. Diagnosis could be made by plain X-ray examination in 38 patients.

    Fifty-seven patients with epistaxis requiring hospitalization during the past eight years were analyzed statistically. The incidence was higher in the middle and older age groups than the younger age group. Such severe epistaxis occurred more frequently in winter and in June. The bleeding site was observed more often in areas other than Little's area (73.7%). They had complications of some systemic diseases, especially hypertens ion (42%). At admission, anemia was seen in 61% of the patients most of whom were males over fifty years old. The duration of hospitalization was less than fifteen days. Half of the cases were treated with Bellocq's tamponade in addition to other conservative therapies. None of the patients required arterial ligation.

    We reviewed 235 patients with laryngeal polyps treated by endolaryngeal microsurgery between January,1971 and December,1985. The patients were divided into 3 groups: those with vocal cord nodule, with vocal cord polyp (localized and diffuse) and with polypoid vocal cord.
    Localized vocal cord polyp accounted for 49% the laryngeal polyps. Vocal cord nodule was more frequently observed in young adult females whose vocal cords were abused. This study suggested that mechanical stimulation, ageing and smoking were etiologic factors in polypoid vocal cord. However, smoking did not appear to be a factor in vocal cord nodule and localized vocal cord polyp. In addition,40.1% of the patients underwent surgery within one year from the onset of symptomes. The shortest duration of symptomes was observed in localized vocal cord polyp. The post-operative improvement rate was high in polypoid vocal cord.

    We described our technique of rhinoplasty via an external approach through transcolumellar and marginal incision, and which allowed cartilage, septum and nasal bone to be visualized for greater surgical accuracy. Three patients with shifted nose,10 patients with posttraumatic septal deviation,3 patients with posttraumatic saddle nose,1 patient with Binder' s syndrome, and 15 patients with cleft lip nasal deformity were operated with this approach and satisfactory results were obtained. Compared with the conventional intranasal approach, our approach has allows better direct vision and easier surgical maneuver. The disadvantages are unsightly columellar scar and more radical surgical maneuver. However, a careful surgical maneuver overcomes these disadvantages.

    An annual survey has conducted nasal symptomes in school children ranging in age from 6-15 years twice a year in spring and autumn for the past 6 years. In children aged 6-9 years, the nasal symptomes appear more often in spring than autumn, while in those age 12-15 years they occurred more often in autumn than in spring.

    We investigated the distribution of Japan cedar pollen in Nagano prefecture from 1983to 1989. Japan cedar began to release their pollens around March and finished release in April. The amount of pollen and the number of days with pollen counted differed with the area measured and calendar year.
    We examined 166 patients with rhinitis by skin tests. The mean age of the 144 patients (86%) who had a postive skin reaction was 26.3 years, whereas it was 51 years in the 22 patients who had a negative skin reaction. House dust was the most commonest (47%)allergen among those tested. Allergic rhinitis caused by house dust, mites and eumycetes began in the teens, and that caused by pollen in the twenties.

    Between 1965 and 1985, sixteen patients eighty years old and over with malignant tumors in head and neck were treated. They consisted of four patients with maxillary cancer, three patients with laryngeal cancer, three patients with tongue cancer, three patients with malignant lymphoma, one patient with parotid cancer, one patient with parotid cancer with malignant melanoma of the nose, and one patient with external ear cancer with maxillary cancer. Most of the patients in whom local disease had been eradicated died within two to three years after the treatment.
    Elderly patients should be treated with consideration of other geriatric affections. None of the patients should reject curative treatment because of their age.

    Therapeutic results of 33 primary cases of maxillary sinus carcinoma treated with irradiation and intraarterial infusion of 5-FU between 1972 and 1984 were analyzed. The 5year crude survival rate for the group with stage T2 carcinoma (n =10) was 50.0%, and for those with T3 (n=15) and T4 (n=8) it was 46.7% and 25.0%, respectively. The overall 5year crude survival rate was 42.4%. Eight patients who did not undergo maxillectomy survived for 5 years after irradiation and intraarterial infusion. Recurrence of the tumor after the irradiation and intraarterial infusion occurred in 63.6%, and was frequently observed at the ethmoidal region and the orbita. In the areas in which the tumor extended to regions such as the ethmoid sinus and orbita, which are nourished by arteries other than the maxillary artery, conventional intraarterial infusion was ineffective for complete tumor eradication. Therefore, in most of the patients with advanced maxillary sinus carcinoma, partial or total maxillectomy following combined therapy of intraarterial infusion and irradiation is necessary to improve a prognosis.

    Forty-three patients with sudden low-tone (<500 Hz) sensorineural deafness (SLTD)were retrospectively studied. Thirty-four patients complained of tinnitus and ear fullness with hearing loss, while nine patients complained of deafness alone. Hearing recovered in about 60% of the patients, including three who showed spontaneous recovery. Recurrent attacks of temporary deafness were noted in six patients.

    A case of malignant lymphoma of the larynx in a 55-year-old male is reported, His chief complaint was hoarseness for six months. Laryngoscopy revealed a tumor in the right false cord. Biopsy showed T cell type non-Hodgkin malignant lymphoma. The patient was treated with chemotherapy (MACOP-B) and radiation therapy (50 Gy). The tumor was well controlled, and no recurrence was noted two months after discharge.

    We reported a patient with findings of Mikulicz's disease and Sjögren's syndrome. A 44-year-old woman who complained of xerostomia and bilateral enlargement of the lacrimal and submandibular glands was referred to our clinic in January,1988. Physical examination, X-ray examination, computed tomography and scintigram revealed remarkable swelling of the lacrimal and submandibular glands and the decreased salivation. Serological examination suggested immunological abnormalities. Histological examination of the right submandibular gland showed marked fibrosis, lymphocytic infiltration and destruction of acini; these findings were compatible with chronic sclerosing sialoadenitis. In March,1989, bilateral swelling of the parotid glands appeared. However, the sialogram of the parotid gland showed normal appearance.

    Six patients with cysts in the posterior paranasal sinuses who compained of visual disturbance are reported. All of them had previously undergone sinus surgery (mean,20.3years). All of them were treated via an endonasal approach under local anesthesia. In five, the visual acuity and the visual field was improved after surgery. However, one did not recover the visual field despite gross improvement in the visual acuity. Computed tomography was very useful to localize the lesion, and the image amplifier was useful to indicate the sphenoid sinus.

    Three cases of parapharyngeal abscess are reported. Case 1 in a 71-year-old male was caused by acute suppurative parotitis. Case 2 in a 60-year-old female was caused by peritonsillitis, suspected to be by anaerobic bacteria. Case 3 in a 75-year-old male was caused by pharyngitis with diabetes mellitus. Case 3 required surgical drainage by the external approach.
    Computed tomography was useful for localizing pathological involvement, and ultrasonography was sensitive for detection of pus within an inflammatory neck mass.

    A case of pendulous tonsil appearing in a 55-year-old woman is reported. The tumor, which was less than 5 mm in diameter 5 years ago, had grown to a size of 2.5×1.6×1.4 cm. Tonsillectomy was performed under general anesthesia. The pendulous position of the tonsil was connected to the upper pole of the left tonsil with steal. Histological findings revealed only tonsillar tissue, with evidence of no malignancy.

    A case of bilateral total deafness after mumps infection is repotred. A 6-year-old boy was referred to us because of sudden hearing impairment in the course of mumps parotitis. Physical examination showed no abnormalities except bilateral parotid swellings. Neurological examination revealed no signs of meningitis or disepuilibrium. The audiogram showed marked bilateral hearing loses at allfrequencies. A diagnosis of sensorineural deafness following mumps was made. High dose of steroid and vasodilators was administered for ten days. However, he did not recover.

    We reported a patient with unilateral arytenoid dislocation complicated with Friedreich ataxia. Conservative treatment with nerve stimulants was followed by closed reduction. Satisfactory results were obtained with this technique, as a result of the early differential diagnosis.

    A case of extramedullary plasmacytoma originating from the epipharynx is reported. A 56-year-old woman was referred to us because of epipharyngeal tumors. The tumors were pediculated and soft. The tumors were removed. Histological examination showed that the tumors consisted of well-matured plasma cells. The cell were found to be neoplastic as they produced a monoclonal ramda light chain. The patient received postoperative radiotherapy. No local recurrence or progression to multiple myeloma was noted after 5 years.

    A case of primary localized amyloidosis causing Mikulicz's syndrome is reported. A 74-year-old female consulted us with compaint of long course of painless swelling of lacrimal glands, parotid glands, submandibular glands and sublingual glands. The initial diagnosis was Mikulicz's syndrome. Prominent progression of the submandibular swelling caused pain during follow-up at our outpatient clinic, therefore, both submandibular glands was removed. A diagnosis of amyloidosis was made.

    Two patients with laryngeal hemangioma are reported. One was a 37-year-old male complaining of frequent hemoptysis. Examination by indirect laryngoscopy revealed a tumor arising from the left arytenoid region. The mass was removed through lateral pharyngotomy, and was found to be a cavernous hemangioma. The second patient, a 41year-old male, was referred to us because of hoarseness. Indirect laryngoscopy showed a sessile tumor from the left vocal cord. The tumor was removed and the histopathological diagnosis was cavernous hemangioma.

    A case of a parotid tumor extending to the parapharyngeal space is reported. A 50year-old female presented with a ten-year history of parotid gland swelling. A small tumor was palpable in the parotid region and diffuse swelling of the left pharyngeal wall and adjacent soft palate was noted. CT-scan showed a large tumor occupying the parapharyngeal space and the oropharynx. MRI revealed that the tumor arose from the deep lobe of the left parotid gland and extended to the parapharyngeal space with a dumb-bell shape. The tumor was removed by an external and oral combined approach. Histological examination of the operative specimen showed a pleomorphic adenoma.

    We reported 2 patients with disseminated intravascular coagulation (DIC) following radiation therapy for maxillary sinus tumor. The first patient died because of the sudden onset and delay of appropriate treatment. Although the second patient subsequently died, DIC was effectively treated with a course of intravenous heparin therapy. As DIC is one of the most serious complications of head and neck tumors, its treatment should be started as soon as possible.

    A 62-year-old man, complaining of left nasal bleeding and left exophthalmos, presented with a tumor in the left nasal cavity. CT scan showed a large mass extending to the leftethmoid sinus, sphenoid sinus and cranium. At the first biopsy, hemangioendothelioma was histologically suspected.
    However, the second biopsy revealed metastatic carcinoma from the kidney. The patient had right nephrectomy 2 years ago under the dianosis of renal cell carcinoma. He was treated with lineac X-ray and interferon. Although the size of the tumor showed little change on CT scans, symptoms such as diplopia, exophthalmos and nasal bleeding were improved.

    A case of dermoid cyst of the mouth floor is reported, a 28-year-old male presented with swelling at the left side of mouth floor and submandibular region. Left sialography revealed upward shift of the left Warton's duct. A cystic mass lesion was found by CT scan and ultrasonography. The cystic tumor was excised through the submental approach. Histological examination revealed a dermoid cyst.
