We reported a case of sphenoidal sinus mucocele showing anterior carvernous sinus syndrome. A 65-year-old male was admitted complaining of severe pain in the trigeminal region and double vision. Examination revealed a mucocele in the both sphenoidal sinus, showing remaining sinuses being intact. Transseptal approach to the sphenoidal sinus was performed. The patient's symptoms completely disappeared after operation. Therefore, we stressed based on the present case the usefulness of microsurgery for sinus operations.
