A case with damage control surgery due to liver and common bile duct injury

Abstract
A 27-year-old male involved in a traffic accident was admitted with a positive Focused Assessment with Sonography for Trauma (FAST) indicating shock. After intubation, he suffered imminent cardiac arrest but was successfully surgery, laparotomy was performed, and gauze packing was applied to the liver injury. Subsequently, Open Abdomen Management (OAM) was implemented. After rewarming and transfusion, re-laparotomy was performed 12 hours later, a complete bile duct transection was diagnosed and treated with liver suturing and external biliary drainage, maintaining OAM. After 38hours, choledocho-jejunostomy was performed, followed by OAM. Despite intestinal edema, closure was achieved through a skin graft procedure. The patient was discharged on the 52nd day, having recovered without complications. This case highlights the efficacy of damage control surgery and early functional reconstruction in saving a patient with a complete bile duct injury.
