Analysis of Postoperative Complications in Acute Cholecystitis According to the New Guideline's Severity Grades

Abstract
Guidelines for the management of acute cholecystitis (AC) were revised in 2013. We analyzed risk factors of postoperative complications according to new guidelines. Participants comprised 255 AC patients who underwent cholecystectomy. All postoperative complications revealed 8.2%. Depends on the guideline severity grades, postoperative complications revealed 0% in Mild, 7.7% in Moderate, and 14.6% in Sever according to the former guidelines. While, these revealed 3.4% in Mild, 11.7% in Moderate, and 50% in Severe according to the new guidelines. Regarding to the risk factor of complications, the factor associated guidelines criterion did not show significant difference in multivariate analysis, however age >72 years and intraoperative blood loss of >90g were significant risk factors. New guidelines severity classification showed more inflammation rather than former guidelines, in addition, age and intraoperative blood loss may represent significant predictors for postoperative complications.
