Abstract
The case is an 81-years-old woman. She had been diagnosed situs inversus from childhood. Because we found anemia when she were hospitalized with pneumonia, we performed gastroscopy and diagnosed duodenal cancer. Then pancreaticoduodenectomy and the Ⅱ A-1 reconstruction was performed. The operative time was 514 minute and the amount of bleeding was 160ml. We could perform conventional pancreaticoduodenectomy uneventfully due to reverse grade image of 3D-CT scan. She had an uneventful postoperative course and was discharged on postoperative day 23. Because there are more vascular mutation with situs inversus than the normal and important vessels are very close in the surgery of the Hepato-Biliary-pancreatic region, it is important that we understand anatomy by using images preoperatively.
