Abstract
Idiopathic interstitial pneumonia (IIP) is a chronic, progressive, and ultimately fatal disease. The prognosis of acute IIP exacerbation is poor and treatment strategies lack standardization. A 47-year-old woman with dyspnea on exertion was diagnosed with IIP. Detailed clinical examination revealed anemia and cecal cancer with synchronous liver metastasis. The patientʼs respiratory condition remained stable on treatment with steroids. Because the patient was a high-risk candidate, we ruled out chemotherapy. Finally, we performed laparoscopic ileocecal resection, and followed up laparoscopic hepatic resection one month later. We chose staged resection instead of simultaneous resection, because the long operative time taken for simultaneous resection posed a serious risk of acute exacerbation of IIP. For such cases, there is little evidence of indication of surgery, courses of treatment, and perioperative management. Herein, we report a case of IIP treated with staged laparoscopic surgery for cecal cancer with synchronous liver metastasis and discuss the relevant literature.
