A Case of Curatively Resected Rectal Cancer with Liver Metastatis and Bladder Invasion Responding to IRIS+C−mab

Abstract
A 45-year-old man introduced to our hospital by diagnosis of rectal cancer. Abdominal CT showed liver metastasis in segment 6 measuring 2.5cm in diameter and a local progressive rectal cancer which invaded the bladder. He was diagnosed of Stage Ⅳ rectal cancer. Since the patient did not want pelvic exenteration, colostomy was performed followed by chemotherapy. After 3 courses of XELOX, tumor marker increased. Since the KRAS gene was wild type, Irinotecan+S-1+cetuximab (IRIS+C-mab) was then administered. After 4 courses of IRIS+C-mab, liver metastasis and primary lesion had decreased in size. Consequently, the patient underwent Hartmannʼs operation and partial resection of the liver metastasis while his bladder was preserved. IRIS+C-mab may be a useful preoperative chemotherapy to decrease progressive rectal cancer in size and to preserve the bladder.
