Two Cases of Pancreatic Serous Cystic Neoplasm Accompanied by Pancreatic Duct Dilation

Abstract

We report two cases of microcystic type serous cystadenoma (SCA). One was a 75-year-old man, and the other was a 54-year-old woman. Both patients complained of upper abdominal discomfort and back pain. CT and MRI revealed cystic tumor around 1 cm in both patients in the pancreatic body and showed dilation of the pancreatic duct just distal to the site of the tumor. Since the dilation of the pancreatic duct suggested invasion of a malignant tumor in both patients, distal pancreatectomy was carried out. The postoperative course of both patients was uneventful. Histopathological examination revealed a microcystic type SCA in both cases. There was no connection between the SCA and the main pancreatic duct. While dilation of the main pancreatic duct was recognized, tumor invasion into the main pancreatic duct of both patients was not revealed. The dilation of the main pancreatic duct was merely due to the compression by the SCA. Although the SCA is a benign tumor, even a small-sized SCA can occasionally cause dilatation of the main pancreatic duct at the distal site of the tumor. In these cases, it was difficult to differentiate SCA from a malignant tumor.
