We report a case of cemento-osseous dysplasia (COD) that appeared to be a multilocular cystic lesion without a high-density region on radiographs. The patient was a 34-year-old woman with a painless and slightly bony swelling in the right molar region of the mandible. Under general anesthesia, the septa of the multilocular cystic lesion were excised. The septa were lined by a thin wall of connective tissue. Microscopic examination of the septa revealed proliferation of fibrous connective tissue accompanied by a mixture of cementum-like globules and bone-like trabeculae without epithelium. A diagnosis of COD was made.
