    Sarcoendoplasmic reticulum calcium transport ATPases (SERCAs) accumulate Ca2+in the endoplasmic reticulum and are essential molecules in calcium-mediated signaling. Some reports indicate that down-regulation of these enzymes is closely associated with carcinogenesis. We thus hypothesized that the ATP2A2 gene, coding SERCA 2 protein, may be associated with oral carcinogenesis. Forty lower gingival squamous cell carcinomas (SCCs), 20 gingival premalignant lesions (leukoplakias), and adjacent normal tissue samples were examined. Immunohistochemical studies revealed strong expression of SERCA2 protein in all normal tissues and significantly down-regulated expression of SERCA2 protein in 45% of SCCs. Expression of SERCA2 in the premalignant lesions tended to decrease, but was higher than that in SCCs. Moreover, analyses according to clinical and pathological classifications indicated that the decreased SERCA2 protein expression in lower gingival SCCs correlated with the progression of carcinogenesis and the extent of malignancy. These results suggested that ATP2A2 gene expression might be altered in relation to the clinical profiles of lower gingival SCCs.

    The concentrations of fluconazole (FLCZ) in serum and mixed saliva after a single intravenous bolus injection of fosfluconazole (504.5mg) were determined by the paper disc method. The samples of serum and mixed saliva were obtained 0.5, 1, 2, 4, 8, and 24 hours after administration. The peak concentrations of FLCZ in serum and mixed saliva occurred 4 hours after administration of fosfluconazole and were 9.45±1.53 and 9.51±2.16μg/ml, respectively. The ratio of the mean concentration ratio of FLCZ in mixed saliva to that in serum was 0.97±0.17. The highest ratio (1.06) was noted 8 hours after administration. There was a significant correlation between the concentration of fluconazole in serum and that in mixed saliva (r=0.8466, p<0.01). A direct relation between the serum and mixed saliva concentrations of FLCZ was thus obtained. The saliva concentration was similar to the serum concentration, indicating that the saliva concentration is a reliable variable for therapeutic drug monitoring (TDM).

    KAI1is a tumor suppressor gene originally identified in prostate cancer. Recent studies of oral cancer, including our investigations, have shown frequent down-regulation ofKAI1expression in many tumor types, whereas mutation of the gene is infrequent. However, the role of the down-expression ofKAI1in oncogenesis and the progression of oral carcinoma remains unclear. In this study, we analyzed the mutational status of KAI1and both the mRNA and protein levels of the gene in a series of gingival carcinomas and precancerous lesions (20 leukoplakias, 50 squamous cell carcinomas, and 20 lymph node metastatic tumors). Although no mutation was found in any sample tested, immunohistochemical studies revealed high frequencies of KAI1 down-regulation not only in the metastatic tumors (20 of 20, 100 %), but also in the primary carcinomas (39 of 50, 78 %) and leukoplakias (9 of 20, 45 %). Down-regulation of KAI1 protein expression was significantly related to carcinogenesis and metastasis (precancerous lesions, primary carcinomas, and metastatic tumors)(p<0.001) as well as to lymph node metastasis (p=0.0022). RT PCR analysis confirmed these results. Therefore, suppressed expression ofKAI1was suggested to play clinically significant roles in carcinogenesis and lymph node metastasis.

    We describe an 8-year girl with atlanto-axial rotatory fixation (AARF) caused accidentally after pharyngeal flap plasty under general anesthesia. On the third postoperative day, she had torticollis without neurologic damage and complained of neck pain when she rotated her head. AARF was diagnosed on the basis of the computed tomographic findings. Glisson traction with 1.5 kilograms of weight for 2 weeks reduced the dislocation, but further treatment was reguired. The deformity was corrected by manipulation under general anesthesia and the patient's head and neck were immoblilized in a Minerva jacket for 6 weeks. A cervical collar was used subsequently in an attempt to maintain normal spinal alignment, and the reduction remained.
    AARF is a rare disorder caused by a variety of conditions. Diagnosis is often difficult and delayed. During operation, hyperextension of the neck should be avoided. It is important to be aware of AARF and to begin treatment as soon as possible in patients with postoperative manifestations such as head tilt, neck pain, and inability to rotate the head.

    We describe a case of intractable cheek phlegmon arising from an odontogenic infection that extended to the interpositional alloplastic material of the temporomandibular joint.
    A 75-year-old man was referred to our department because of a painful swelling in the right temporal region occurring after an acute odontogenic infection of right upper molar teeth. The past history revealed reconstruction surgery with iliac bone transplantation following a right hemimandibulectomy to treat an ameloblastoma about 26 years ago. Physical examination and imaging findings of CT and MM revealed a diffuse swelling in the temporobuccal region and a foreign body in the temporomandibular joint region. The clinical diagnosis was temporal cellulitis from an infection involving the interpositional alloplastic material of the temporomandibular joint. Initial chemotherapy was effective, and the swelling gradually disappeared. However, a fistula formed at the buccal region and pus discharge continued in spite of conservative therapy with irrigation. Under local anesthesia, we carried out curettage of the fistula two times, and the silicone-like foreign body, which might have been implanted at mandibular reconstruction surgery, was removed. Then, the symptoms of the infection rapidly and completely disappeared.

    We report a case of mandibular fracture combined with hyoid bone fracture and arytenoid dislocation caused by injury. The patient was a 28-year-old man who was assaulted by several persons at a shopping quarter at midnight. He was unable to breathe or speak for a while, but these symptoms disappeared while he was rubbing his neck. He was transported by ambulance to our hospital. There was no dyspnea, and he could speak. He complained of swallowing pain and trismus. A fracture line was found in the left side of the coronoid process of the mandible on radiography. CT imaging showed a fracture in the right great horn of the hyoid bone, with inflammatory edema around the hyoid bone and epiglottis. Moreover, the arytenoid cartilage had been bilaterally dislocated posteriorly. Closed reduction and fixation was performed to treat the mandibular fracture, and the laryngeal injury was observed. Laryngeal injuries, even in the absence of symptoms or functional disorders immediately after injury, can later cause airway obstruction. Careful diagnosis is therefore necessary.

    We report a case of cemento-osseous dysplasia (COD) that appeared to be a multilocular cystic lesion without a high-density region on radiographs. The patient was a 34-year-old woman with a painless and slightly bony swelling in the right molar region of the mandible. Under general anesthesia, the septa of the multilocular cystic lesion were excised. The septa were lined by a thin wall of connective tissue. Microscopic examination of the septa revealed proliferation of fibrous connective tissue accompanied by a mixture of cementum-like globules and bone-like trabeculae without epithelium. A diagnosis of COD was made.

    We report a case of recurrent ameloblastoma arising in the right side of the mandible with pulmonary metastasis occurring 15 years after the last operation. The recurrent tumor was located in the right parotid-masseter region and metastasized into the right lower lobe of the lung. The mandibular and pulmonary tumors were both excised. The excised tissues showed the pattern of a follicular ameloblastoma, with no cytological features of malignancy in either the mandibular or metastatic lesion.

    We report a case of rhabdomyosarcoma (RMS) that was successfully treated by combined chemotherapy with cisplatin and caffeine. This case was characterized by resistance to treatment with cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) chemotherapy and irradiation of carbon ions by an heavy ion medical accelerator in Chiba (HIMAC).
    We describe an 18-year-old man with a local recurrent RMS that developed after CHOP chemotherapy and HIMAC irradiation. The patient had undergone partial resection for a recurrent huge tumor near the pterygopalatine fossa. Tumor proliferation occurred after the first operation; therefore, partial resection was planned again. Cisplatin combined with caffeine was given as preoperative chemotherapy for the second partial resection because CHOP chemotherapy had been ineffective. The RMS responded remarkably to cisplatin combined with caffeine, both macroscopically and histologically. The treatment was adequately effective as preoperative chemotherapy.
    Although the prognosis of RMS has improved since several treatments have become available, the management of some types of RMS remains challenging. Cisplatin combined with caffeine chemotherapy should be considered as one treatment option in such cases.

    Malignant melanoma in the oral cavity is rare lesion frequently associated with lymph node metastasis, distant metastasis, or both at an early stage.
    A 75-year-old man was referred to our department for the treatment of primary malignant melanoma of the maxillary gingiva, diagnosed after lobectomy for a metastatic lung tumor. He underwent partial maxillectomy including the primary tumor, combination chemotherapy with DTIC, ACNU, and VCR, and nonspecific immunotherapy with OK-432.
    As of 3 years 8 months after the start of treatment, neither recurrence nor metastasis has occurred. The outcome has been fairly good, and the patients' quality of life has been maintained.

    Hemostasis was attempted by transcatheter arterial embolization (TAE) in patients with malignant oral tumors for whom radical therapy was difficult. The procedure was done by our hospital's radiology department. This study showed that when pseudoaneurysms or culprit vessels could be identified by angiography, satisfactory hemostasis was achieved with the use of a permanent embolization agent.However, when the tumor was rich in feeding vessels and peripheral vessels involved in bleeding could not be identified, a temporary embolization agent was injected. This procedure failed to achieve satisfactory hemostasis. Nonetheless, as compared with surgical ligation, TAE is much less invasive, and the temporary achievement of hemostasis is clinically significant.

    Several cases of ossification or calcification of the articular disc associated with temporomandibular joint synovial osteochondromatosis have been reported previously. However, synovial osteochondromatosis accompanied by ossification of nearly the entise disc is extremely rare.
    A 58-year-old man was referred to our department because of pain in the left temporomandibular joint. Radiography showed three radiopaque areas in front of the head of the mandibular condyle head. Diagnostic arthroscopic surgery revealed a perforated articular disc and a hyperplastic synovial membrane in the left temporomandibular joint. We carried out open surgery. When the joint cavity was opened, a loose body was seen in the border between the lateral pterygoid muscle and the articular disc. Histopathologically, the articular disc was nearly completely ossified. We diagnosed synovial osteochondromatosis.

    We encountered a patient with a blood boil in the maxillary sinus, and observed the time-course of its development by MRI. The patient was a 70-year-old woman with a chief complaint of right epistaxis.
    Comparison of MM images obtained three times in 5 years showed that the mass had enlarged. Since imaging findings suggested a malignantt umor, we performed a biopsy, which revealed a mass of clotted blood. fter removal of the mass under general anesthesia, histopathological examination demonstrated a hematoma, but no neoplastic lesions.

    We performed eminectomy after arthroscopic examination in a patient with chronic dislocation of the temporomandibular joint (TMJ). The operation was done under general anesthesia. Preoperative Tl-weighted magnetic resonance images showed that the articular disc was located anteriorly to the eminence. There was no evidence of deformation. The mandibular condyle was displaced more anteriorly than the disc. On T2-weighted images, joint effusion was bilaterally present around the mandibular fossa in the upper joint space. Arthroscopic examination of the upper joint space revealed fibrous adhesive lesions in the anterior and posterior synovial space. Removal of the fibrous adhesive lesions after opening the TMJ and eminectomy enabled manipulative reduction. There was no relapse of dislocation after operation, and the outcome was good.
