We report a rare case of primary malignant melanoma of the parotid gland in a 67-year-old man. Puncture aspiration biopsy of the parotid gland revealed a malignant melanoma. Positron emission tomography computed tomography suggested cervical lymph node metastasis. We performed parotidectomy, skin excision, and left cervical lymph node dissection. The patient underwent reconstruction using a superficial circumflex iliac artery perforator flap, with resection limited to the cervical branch of the facial nerve. Histopathological examination revealed no lymph node metastases. Eleven months postoperatively, we resected a metastatic lesion detected in the small intestine. The reconstructed skin flap had a good appearance and contour; however, its color slightly differed from the surrounding skin and mild paralysis of the depressor labii inferioris persisted. Primary malignant melanoma of the parotid gland is associated with poor prognosis and warrants careful follow-up.
