A congenital cheek fistula is a rare malformation characterized by an external opening on the cheek. It is classified into two types: the simple cheek fistula, which lacks communication with the parotid gland, and the parotid duct fistula, which is connected to the parotid gland. We report the case of a 3-year-old girl with a left-sided congenital cheek fistula and recurrent infections. After resolving the inflammation, the patient was eligible for radical surgery. To our knowledge, this is the first reported case of a pediatric congenital cheek fistula with infection in Japan. This case underscores the importance of considering parotid duct communication when diagnosing and surgically treating congenital cheek fistulas.
