Abstract
A 10-year-old boy was admitted to our hospital with a persistent cough and general fatigue. Complete atelectasis of left lung was observed on chest X-ray film and CT scan. Six months before persistent cough, he was diagnosed as ulcerative colitis. Then he kept taking mesalazine. When we conducted bronchofiberscopy, huge quantities of bronchial casts were extracted from the left main bronchus till lobe.Because of prolonged hypoxic state, he needed mechanical ventilator for 5 days. It is difficult to differentiate between pulmonary involvement of ulcerative colitis and side effects of mesalazine treatment. His drug-lymphocyte stimulation test with mesalazine was positive. There was a possibility that pulmonary injury in this patient were caused by allergic reaction for mesalazine.
