Abstract
The case was a 68-year-old man. The chief complaint was distention of the left flank. He received a trauma with multiple costal fractures, left hemopneumothorax, minor splenic laceration, fractures of the left tibia and fibula by a traffic accident on June, 2006. Three months after the accident, he complained distention of the left flank when he was in our hospital for rehabilitation. We diagnosed it as traumatic abdominal wall hernia by the abdominal CT. Traumatic abdominal wall hernia is rare, and only 11 cases have been reported in Japan. Eight cases were caused by traffic accidents and 5 cases of them suffered costal or pelvic fractures. Interestingly, all of the 5 cases with fractures hernia occurred several months after the trauma. We found correlation between fractures and delayed onset. We repaired the hernia with PHS (PROLENE Hernia System). In our knowledge, this is the first report to repair traumatic abdominal hernia by PHS. This method is reasonable and effective, because PHS can cover the wide area of the peritoneum with a underlay-patch, closing hernia gate with the connector-parts, which fixes the system with onlay-patch sutured on the fascia.
