Abstract
An 83-year-old woman was admitted for treatment of cholecystocholedocholithiasis. Abdominal CT scan and US revealed several 1.0-cm gallstones and two 0.7-cm stones in the common bile duct. Due to the presence of a duodenal diverticulum, the common bile duct (CBD) could not be cannulated selectively ; endoscopic sphincterotomy (EST) could not be done. During a laparoscopic cholecystectomy (LC), a C-tube was inserted in the CBD via the cystic duct 2 days postoperatively. EST of the papilla was done via a guide wire inserted by the transcystic route for the removal of CBD stones. Three days postoperatively, the C-tube was removed after cholangiography. EST using a C-tube inserted during LC is useful for the removal of CBD stones when endoscopic bile duct cannulation cannot be performed.
