Abstract
A 75-year-old man was treated by pancreatoduodenectomy with modified Child's reconstruction for recurrence of mucinous carcinoma 17 months ago. Postoperative chemotherapy was interrupt by cachexia due to local recurrence and lung metastasis. An enlarged abdominal mass caused afferent loop obstruction, jaundice and cholangitis, so percutaneous bowel drainage (PBD) was performed for palliative care. The dilated loop of jejunum was drained percutaneously with a 12Fr aspiration tube via the tract of the external pancreatic duct stent. Cholangitis, upper abdominal pain and anorexia were all improved immediately without complication by PBD. The external drainage did harm his quality of life, so a metal stent was placed and the external drainage tube was removed. PBD might be a very safe and effective procedure for malignant afferent loop obstruction following a pancreatoduodenectomy.
