Abstract
We report on a case of a 59-year-old Japanese female patient diagnosed with intrahepatic cholangiocellular carcinoma with a sarcomatous change. An abdominal computed tomography confirmed a low-density mass, 10 cm in longer diameter, with enhancing peripheral areas. Positron emission tomography showed abnormal FDG uptake (SUV max=15.30) in the tumor. The tumor was considered to have high malignant potential. Extended lateral segmentectomy of the liver was thus performed. Histological examination of the tumor showed poorly differentiated cholangiocellular carcinoma with a sarcomatous change. Immunohistochemistry of the tumor showed positive staining for cytokeratin 7 and negative for vimentin and cytokeratin 19. The labeling index of Ki67 was high (28%) that might reflect increased cellular proliferative potential of the tumor. The patient's postoperative course was uneventful. And now, she is doing well one year after the surgery under adjuvant chemotherapy with GEM. The literature on this rare disease was reviewed and discussed.
