Abstract
We report a case of femoral hernia with incarceration of the appendix. A 70-year-old woman was admitted to hospital because of a right inguinal tumor complicated by pain. Physical examination revealed a mass, about 3 cm in diameter, just caudal of the right inguinal ligament. An emergency operation was carried out with a diagnosis of right incarcerated femoral hernia. The appendix was incarcerated in the femoral ring and became necrotic. Therefore, an appendectomy and a hernioplasty using McVay's procedure were performed via the same surgical incision. Cases with an incarcerated femoral hernia containing an appendix are rare. A review of the Japanese literature shows that individualized treatment is carried out in such cases ; critical complications such as a contaminated mesh have not been reported. Currently, tension-free surgical procedures are used to treat inguinal hernias easily. However, anatomical knowledge is necessary to carry out sutured surgical procedures for cases which are unsuitable for tension-free surgical procedures.
