Abstract
We report a case of laparoscopy-assisted total gastrectomy for juvenile gastric polyposis. Our patient was a 28-year-old woman with a family history of intestinal polyposis from her maternal grandfather, mother and older brother affected. Endoscopic findings of the upper gastrointestinal tract revealed edematous polyposis in the middle to lower third of the stomach. Surgery was planned due to polyp-related leaking anemia and hypoproteinemia which were resistant to internal treatment. Mini-laparotomy (5 cm in size) with five ports, using the pneumoperitoneum method, laparoscopy-assisted total gastrectomy and Roux-en-Y reconstruction were performed. The total operative time was 257 minutes and there was no blood loss. Her postoperative course was uneventful, and she was discharged on the 10th postoperative day. Histological examination showed mixed juvenile and hyperplastic polyposis, and sequencing analysis identified a pathogenic variant in exon 11 of the SMAD4 gene. Laparoscopic surgery was cosmetically preferable for this treatment of juvenile gastric polyposis.
