Endovascular Treatment of Isolated Iliac Artery Aneurysm by Superficial Femoral Approach Complicated with Lymphatic and Vascular Malformation

Abstract

We successfully treated a patient who presented with left external iliac artery aneurysm complicated with serious congenital lymphatic and vascular malformation using the endovascular technique with an Excluder contralateral leg inserted from the left superficial femoral artery. A 66-year-old woman who suffered from left lower limb venous stasis syndrome and recurrent thrombophlebitis from youth was revealed to have a left external iliac artery aneurysm 43 mm in diameter. Preoperative computed tomography (CT) revealed multiple lymphatic and venous malformations accumulated diffusely from the left pelvic cavity to the left inguinal region and hyper-vascular overgrowth in the left foot. We performed iliac leg stent graft insertion from left superficial femoral artery followed by coil embolization of the left deep circumflex iliac artery. She was discharged on post-operative day 7 without any complications, and no endoleak was observed on follow-up CT after 1 month.
