A Case of Severe Sepsis due to Aeromonas Punctata Infection after the Operation for Afferent Loop Syndrome

Abstract
Although the genus Aeromonas consists of gram-negative rods which are generally low virulent, they can cause severe septicemia in patients with an underlying disease such as liver cirrhosis or diabetes. There are few reports describing that the genus Aeromonas caused postoperative infections so far. We present a case of severe sepsis due to Aeromonas punctata infection followed a dramatic course after the operation for afferent loop syndrome.
The patient was a 64-year-old man who underwent an operation under the diagnosis of afferent loop syndrome. Laboratory examination revealed remarkable decreases in the white blood cell and platelet count and an elevation in the creatine phosphokinase level two days after the operation. Purpura and swelling also developed in the right inguinal region and spreaded rapidly. A biopsy of the subcutaneous soft tissue at the right femur revealed gram-negative rods. Debridement of the bilateral lower extremities from the trunk was performed three times under the diagnosis of postoperative necrotizing soft tissue infection. Aeromonas punctata was isolated from the blood, ascites and resected tissue. The patient conquered postoperative anastomosis leakage and was discharged on independent gait on 110th day after the operation.
