Abstract
An 86-year-old woman presented with a complaint of deformity and stiffening of the left breast. She was diagnosed to have a left breast carcinoma, cT2N0M0 stage IIA, ER : 3b, PgR : 2, and HER2 : 1+. Owing to her advanced age, she did not desire any invasive treatment, and therefore we started hormonal treatment. Three months after administration of anastrozole, a temporary reduction in tumor size was observed, but 7 months after initiating treatment, the tumor was found to have enlarged ; therefore, we changed the treatment to fulvestrant (FUL). After administration of FUL, the tumor entered a state of stable disease (SD) and has been in SD for a year. According to the breast carcinoma treatment guidelines, surgical resection is standard treatment regardless of patient age. However, many patients do not agree to surgical treatment because of their advanced age, coexisting diseases, views on life and death, and other reasons. In conclusion, we administered FUL to an elderly patient with breast cancer who did not consent to surgery. SD was achieved over a long term.
