Abstract
A 67-year-old man had undergone endovascular aneurysm repair (EVAR) for impending rupture of an abdominal aortic aneurysm (AAA) 3 years previously. The aneurysm began to enlarge again 2 years after surgery. No contrast flow into the aneurysm (endoleak) was noted, but the aneurysm continued to enlarge (endotension). Since the aneurysm had enlarged 2 cm by 3 years postoperatively, elective open abdominal surgery was planned. However, while waiting for surgery, the patient had an AAA rupture with shock due to stent graft migration and was transported to our hospital by ambulance. Emergency iliac artery stent grafting was successfully performed. This case of life-saving emergency surgery in a patient with late aneurysm rupture during follow-up of endotension after EVAR is reported.
