Abstract
A 61-year-old woman underwent left total mastectomy + sentinel lymph node biopsy in her previous hospital for the diagnosis of left breast cancer. Although postoperative chemotherapy was initiated, as the cancer was pathologically stage II A and HER2(3+), treatment was interrupted because of her mental and economical issues. Finally, after 3 courses of fluorouracil, epirubicin, and cyclophosphamide and 1 course of docetaxel and trastuzumab in a previous hospital, 17 courses of trastuzumab were administered in our clinic, without either recurrence or complications.
There exist many barriers for habitants who live on remote islands for treatment. Chichijima island is located 1,000 km away from Honshu-main island. The only means of transportation is by ship, the journey of which takes 24 hours and departs every 6 days, and our clinic is the only medical institution for the island. We must accommodate patients' backgrounds adequately to decide treatment plans. It can be a choice to undergo anti HER2 therapy in remote island clinics based on sufficient consideration.
