A Case of Malignant Phyllodes Tumors of the Breast with Metastasis to the Pancreas

Abstract
The patient was a 44-year-old woman diagnosed as having malignant phyllodes tumors of the breast. Surgical treatments, including bilateral mastectomy and breast reconstruction, were performed for locally recurrent bilateral malignant phyllodes tumors of the breast. One year after the surgery, a metastatic lesion appeared in the upper lobe of the right lung and chemotherapy was started. However, a new lesion was subsequently found in the lower lobe of the right lung, and partial resection of the right lung was performed. Six years after the first surgery, the patient was brought to the hospital by ambulance because of acute left upper abdominal pain. Imaging examinations revealed a tumor in the pancreatic tail. Distal pancreatectomy was performed. Histopathological examination of the resected specimen showed metastatic phyllodes tumor of the pancreas. Malignant phyllodes tumor is said to be associated with distant metastasis in 26% of cases, but the metastases are mainly found in the lung, pleura, bone, etc ; metastasis to the pancreas is extremely rare. The patient presented herein needed several surgical procedures for repeated local recurrences and distant metastases, which may have contributed to her survival. Therefore, careful monitoring is important for patients with malignant phyllodes tumors.
