We herein report an extremely rare case of ileal duplication incarcerated into inguinal hernia, which was diagnosed with laparoscopy. A 69 -year-old man was referred to our hospital because of right inguinal swelling. He was diagnosed as right inguinal hernia, and transabdominal preperitoneal hernia repair (TAPP) was attempted. Laparoscopic observation revealed the ileal duplication incarcerated into inguinal hernia. The ileal duplication was dissected from inguinal hernia and peritoneum was closed by laparoscopic procedure. The ileal duplication was resected from small laparotomy. Right inguinal hernia was repaired by anterior approach with mesh-plug. The laparoscopic-assisted surgery was considered to be a very useful method for this diagnosis and treatment. We reported this extremely rare case with literatures.
