A 72-year-old woman visited our department after being found with masses in the presacral space of the retroperitoneum by abdominal CT during a detailed examination of an inguinal atheroma. Abdominal CT revealed mass lesions measuring 66 mm, 48 mm, and 10 mm, in the pelvis. Although no distant metastasis was found, they were judged to be multiple mass lesions that were highly malignant, and the patient was suspected to have retroperitoneal liposarcoma. She underwent laparotomy to remove the masses. The pathological diagnosis indicated extra-adrenal myelolipoma and the patient was diagnosed with presacral myelolipoma. The patient is alive without recurrence as of 4 years and 7 months after the surgery. No cases of multiple extra-adrenal myelolipoma have been reported in Japan. Herein, we report such a rare case.
