An 84-year-old man presented with right lower abdominal swelling. Preoperative computed tomography imaging revealed an internal inguinal hernia. Laparoscopic inguinal hernia repair was performed, and a right internal inguinal hernia was detected, as diagnosed preoperatively. However, unlike an external inguinal hernia, a hernia defect appeared to have developed in a different direction from the inguinal canal. The patient was diagnosed with interstitial interparietal hernia. Due to a coexisting spermatic cord lipoma, conventional laparoscopic inguinal hernia repair was performed with dissection of the spermatic cord lipoma. This case of inguinal interparietal hernia and spermatic cord lipoma is useful for transabdominal preperitoneal repair in terms of diagnosis and treatment.
