A 50-year-old man presented to our hospital in December 2021 with bulging and severe pain in the right inguinal region that appeared a few days earlier. Inguinal hernia was suspected but was not confirmed by computed tomography. As the patient strongly requested surgery, transabdominal preperitoneal repair (TAPP) was planned. Laparoscopic examination of the abdominal cavity revealed right cryptorchidism and confirmed the inguinal hernia, which was classified according to the Japan Hernia Society as type L1. After consulting with our urologist, we decided to performe operation in combination with the urologists at a later date. In May 2022, the patient underwent laparoscopic orchiectomy and TAPP. Postoperative course was good, and the patient was discharged on the second postoperative day. Cases of adult inguinal hernia with cryptorchidism are rare in Japan. In addition to the case report, the relevant literature was reviewed.
